Treatment
Desiccated thyroid extract delivers both T4 and active T3 in one medication. For patients who never quite felt right on levothyroxine alone, it can be the missing piece.
Overview
Natural desiccated thyroid (NDT), also called desiccated thyroid extract (DTE), is a prescription thyroid medication derived from porcine thyroid. Unlike levothyroxine, which supplies only T4, NDT contains both T4 and the active T3 hormone in a fixed ratio. Common brands include Armour Thyroid and NP Thyroid.
Who It Helps
Most people do fine on levothyroxine. But a meaningful subset continues to have hypothyroid symptoms — fatigue, brain fog, weight that won't budge — despite a 'normal' TSH on T4. Often these patients convert T4 to active T3 inefficiently. Because NDT supplies T3 directly, it can relieve symptoms that T4 alone leaves behind. The same logic applies to T3/T4 combination therapy.
Safety & Monitoring
NDT is effective but requires careful dosing and follow-up, because the added T3 acts faster than T4. We start conservatively, recheck a full panel including Free T3, and adjust to your symptoms and labs. It isn't right for everyone — particularly some patients with heart conditions — which is why treatment is individualized and monitored. Your clinician will weigh the options with you based on your complete thyroid panel.
