Botox vs. Daxxify: Onset, Duration, Dosing, and Cost-Per-Month Math (2026 Guide)
Mechanism, onset, duration, dosing, and cost — an honest, clinically-grounded comparison of Botox and Daxxify from Naya Wellness’s medical team.
If you are weighing Botox against Daxxify for frown lines or forehead creases, you are essentially choosing between the longest-running gold standard in cosmetic neuromodulators and the longest-lasting one approved in the US. Both are FDA-approved type A botulinum toxins. What differs is duration, dosing, formulation, and what your annual budget looks like.
This guide walks through every meaningful difference based on the current FDA labels, the SAKURA phase-3 trials, and what we see day-to-day at our Chatsworth and Santa Barbara clinics. We also do the cost-per-month math, because Daxxify costs roughly twice as much per treatment as Botox — and lasts roughly twice as long.
Quick Take
Daxxify lasts substantially longer than Botox — a median of about 6 months in the SAKURA trials, with a meaningful subset of patients seeing effect at 9 months — versus a median of 3–4 months for Botox. Botox is the older, more universally familiar product with two decades of cosmetic use behind it and predictable per-treatment pricing. Daxxify costs roughly $560–$800 per glabellar treatment vs. $280–$320 for Botox, but when you divide by months of effect, the monthly cost is closer than people assume. Choose Botox for predictability and lower per-visit cost; choose Daxxify if you genuinely want to treat less often and are comfortable with the higher upfront spend.
What Botox and Daxxify Actually Are
Both products are purified type A botulinum toxin. Both work by temporarily blocking acetylcholine release at the neuromuscular junction, which silences the targeted facial muscle and lets the overlying skin smooth out.
Where they differ is in the formulation surrounding the active toxin:
- Botox (onabotulinumtoxinA) — made by Allergan Aesthetics (an AbbVie company), FDA-approved for cosmetic glabellar lines in 2002. It is formulated with a roughly 900 kDa accessory-protein complex and human serum albumin as a stabilizer.
- Daxxify (daxibotulinumtoxinA-lanm) — made by Revance Therapeutics, FDA-approved for cosmetic glabellar lines in September 2022. Daxxify is formulated with a proprietary peptide excipient called RTP-004 instead of human serum albumin. Revance’s hypothesis is that this peptide helps the toxin bind to nerve terminals more efficiently, which appears to translate into the longer observed duration in clinical trials.
Both are excellent at silencing facial muscles. The interesting clinical story with Daxxify is not the mechanism — it is the duration data.
Onset: When Will You See Results?
Onset is comparable, with Daxxify modestly faster on average.
- Botox onset: Most patients see softening at 3–7 days, peak effect at 10–14 days.
- Daxxify onset: SAKURA data show many patients responding within 1–4 days, peak around 14 days.
The onset gap is small enough that most patients do not notice it. If you have an event in 7–10 days, either product will likely be at or near peak. Onset is rarely the right reason to pick between these two.
Duration: The Headline Difference
This is the entire point of choosing Daxxify.
- Botox: Typical duration is 3–4 months for glabellar lines. The FDA label specifies that re-treatment should not be more frequent than every 3 months.
- Daxxify: The SAKURA 1 and SAKURA 2 phase-3 trials reported a median duration of approximately 6 months at glabellar 40-unit dosing, with a subset of patients still showing investigator-rated improvement at 9 months.
That is roughly double the duration of every other FDA-approved cosmetic neuromodulator currently on the market. For patients who hate the “wearing off” feeling at month 3, or who would prefer to come in twice a year instead of four times, Daxxify is the only product in the category that delivers that experience.
Two honest caveats. “Median 6 months” means roughly half of patients last shorter and half last longer — some patients do not get a meaningfully longer result with Daxxify than with Botox. And individual variables — metabolism, muscle mass, dose adequacy, technique — still matter regardless of which molecule is in the syringe.
Dosing and the 2:1 Conversion Ratio
Daxxify units are not the same as Botox units. Per the FDA labels:
- Botox for glabellar lines: 20 units total, divided across 5 injection sites (4 units each).
- Daxxify for glabellar lines: 40 units total, divided across 5 injection sites (8 units each).
The widely used clinical conversion is approximately 2 units of Daxxify ≈ 1 unit of Botox in cosmetic doses. As with every other neurotoxin comparison, the FDA labels explicitly warn that units are not interchangeable between products, so your injector should recalculate the dose from scratch when switching you between them rather than substituting unit-for-unit.
When you compare prices between clinics, never compare per-unit price alone. The Daxxify dose for the same treatment area is roughly twice the unit count, so a “low” per-unit price can still translate to a higher per-treatment total. Per-treatment cost is the only fair comparison.
Diffusion and Precision
Both Botox and Daxxify have tight, predictable diffusion profiles. Daxxify’s peptide-stabilized formulation behaves similarly to Botox in terms of localization, which makes it a good fit for the same focal targets — crow’s feet, lip flips, masseters — that experienced injectors already treat with Botox.
If you have heard that Daxxify “spreads more” or “spreads less,” the head-to-head data do not strongly support a meaningful difference. Choose between them on duration, cost, and visit frequency, not on diffusion mythology.
Cost: What You Will Actually Pay in 2026
National 2026 averages, with the usual caveat that prices vary by city, injector experience, and treatment area:
- Botox: roughly $12–$20 per unit, with most clinics in the $14–$16 range. A standard glabellar treatment (20 units) lands around $280–$320.
- Daxxify: roughly $14–$20 per unit. A standard glabellar treatment (40 units) lands around $560–$800.
Per visit, Daxxify is roughly twice the cost. But here is the math that actually matters for your annual budget.
If Botox lasts 4 months, you need 3 treatments per year, for an annual cost of roughly $840–$960. If Daxxify lasts 6 months, you need 2 treatments per year, for an annual cost of roughly $1,120–$1,600. On a per-month basis, Botox runs roughly $70–$80/month and Daxxify roughly $93–$133/month. Daxxify is more expensive annually for most patients, but the gap is smaller than the per-visit price tag suggests, and the trade is one fewer visit per year plus smoother continuous effect.
For some patients, treating twice a year instead of three or four times is worth the spread. For others, paying less per visit and treating more often makes more sense.
Side Effects and Safety
The side effect profiles for Botox and Daxxify are very similar, as is true of every type A botulinum toxin. In the SAKURA trials and post-marketing data for Daxxify, the most common adverse events were:
- Headache (3–6%)
- Injection-site reactions — bruising, redness, mild swelling (1–5%)
- Eyelid heaviness or ptosis (uncommon in experienced hands)
- Asymmetric brow position, typically correctable at a 2-week touch-up
One trade-off specific to Daxxify worth understanding: if a result is suboptimal — say, a brow that lands a touch heavier than intended — it lasts roughly twice as long as the equivalent Botox result would. That is a strong argument for choosing an experienced injector specifically for Daxxify, and for considering Daxxify only after you already know how your face responds to neuromodulators in general.
Both products carry the same FDA boxed warning about the theoretical possibility of distant toxin spread. In cosmetic facial doses, this has not been a clinically meaningful concern across millions of treatments.
Who Is the Ideal Candidate for Each?
Botox may be the better fit if:
- You are a first-time neuromodulator patient and want to learn how your face responds before committing to a longer-acting product
- You prefer lower upfront cost per visit
- You like the option to fine-tune between treatments — heavier on the forehead one cycle, lighter the next
- You have responded predictably to Botox in the past and have no reason to switch
Daxxify may be the better fit if:
- You have used Botox or Dysport before and consistently feel them wearing off at month 3
- You travel often, have a busy schedule, and want fewer clinic visits
- You value continuous effect over the ability to adjust frequently
- You are not albumin-sensitive and prefer a formulation without human serum albumin
Can You Switch Between Botox and Daxxify?
Yes. There is no medical reason you cannot move from one to the other between treatments. A few practical notes:
- Tell your new injector which product you previously had, the dose, and when. This guides dose calculation.
- You cannot combine the two products in the same treatment area in the same session.
- When switching from Botox to Daxxify, most clinicians wait until Botox has substantially worn off (typically 3+ months) before dosing Daxxify, so they can assess where the new product needs to work.
- Switching does not reduce the small theoretical risk of antibody formation against either toxin.

Botox vs. Daxxify FAQ
The SAKURA 1 and SAKURA 2 phase-3 trials reported a median duration of approximately 6 months at the 40-unit glabellar dose, with some patients showing investigator-rated improvement at 9 months. “Median” means roughly half last longer and half last shorter, so your individual result may vary.
It depends on how you value clinic visits versus per-visit spend. On an annual basis, Daxxify is more expensive than Botox for most patients, but the gap shrinks once you factor in the reduced number of visits. If treating twice a year instead of three or four times is meaningful to your schedule, Daxxify can be worth the spread.
Most patients describe the in-treatment experience as essentially identical — same injection feel, same recovery, same look at week 2. The noticeable difference shows up at month 4 or 5, when Botox would typically be wearing off and Daxxify is still working.
The published safety profiles are very similar. The one practical consideration with Daxxify is that any side effect — including a brow that lands slightly heavier than intended — also lasts roughly twice as long. That argues for using an experienced injector and ideally for having tried a shorter-acting product first.
Daxxify is FDA-approved specifically for glabellar lines, but is commonly used off-label by experienced injectors for forehead lines and crow’s feet, similar to how Botox was used off-label for these areas before its expanded FDA indications. Discuss your specific treatment area with your provider.
No. Daxxify uses a proprietary peptide excipient (RTP-004) in place of human serum albumin. This is occasionally relevant for patients with specific concerns about albumin-containing formulations.
Standard post-injection guidance is the same as for Botox: avoid strenuous exercise, lying flat, and rubbing the treated area for approximately 4 hours after injection.
